Benefits are effective 01/01/2019. You may also downloadthe app "Health4me". You will need to register using your SS# as the ID number with Group/PLan #915911. Part of the app allows you to view your card.

    The 2019 Active Firefighter Standard Life Insurance form. If you are not increasing coverage you do not have to do anything. You may increase your coverage by $30,000 without medical screening.     It is with heavy hearts and great sadness that Local 1403 announces the passing of our brother, President Emeritus Dominick F. Barbera at the age of 84 on September 28, 2018. Dominick "Dom" began his distinguished career with the Metro-Dade Fire Department in 1966 and served our community for 30 years when he retired as Lieutenant in 1996. He dedicated his life to Local 1403 and the I.
